MAF VENDOR REGISTRATION FORM

Thank you for your interest as a vendor for MAF! Most of MAF activities are produced free of charge or at very low cost to its participants and the general public. Our goal is to provide quality family activities for youth and adults interested in promoting discipline, positive attitudes and self-esteem with personal accountability through martial arts programs. Your participation as a vendor will help support this effort with your MAF vendor fee donation.
Vendor must agree to ALL of the following. • $50 deposit towards total vendor fee. • Must have own power source, set up materials and equipment. • Vendor must clean up and remove all waste and materials generated from operation. • Vendor must limited the sale or promotion of goods and/or services to items specifically submitted by the vendor in the vendor application form and approved by MAF.
On average, vendor donations range from $75 - $300, depending on the type of vending service you wish to offer. Some vendors may qualify for a reduced vendor fee while other vendors may be asked to contribute a fee exceeding the $300 range.
A basic guideline of the vendor fees are as follow. • Primary Food Service: $300-$1000 • Secondary Food-Service: $75-$200 • Professional Service Promotion: $50-$300 • Retail Business: $150-$300 • Nonprofit organizations: $50-$150
Note: the above fees are not absolute but merely a guideline of a sliding scale. Vendors cannot deviate from any items listed in the application form without the express permission of MAF. All vendor fees are subject to change without notice.
A minimum upfront deposit of $50 is required upon submission of your application, which is applied to the vendor fee sum total if your application is approved. To avoid vendor cancellation you will be required to remit the remaining balance within (5) days after approval notification is given or no later than (2) days prior to the scheduled event, whichever comes first. If your application is not approved you will be entitled to a refund (minus a $10 application review fee). If your application is approved but later canceled for nonpayment of the vendor fee balance will constitute a forfeiture of all vendor fees collected.
Possible reasons for vendor application denial may include but not be limited to: vendor duplication of goods or services; limited vendor space available; goods or services may be viewed as offensive for the participating public or community; vendor fees were not paid in full at the designated time.
